I, on behalf of the parents or guardians of the children in my care, acknowledge that I HAVE THEIR PERMISSION TO ATTEND AND PARTICIPATE in a party at DreamLand.
I understand that there is IMPLIED RISK (kids can be hurt) of injury due to the nature of children at play.
ENGAGED SUPERVISION of the children in my care is my responsibility as party host. I will provide adequate supervision to all in my care.
I WILL PROVIDE supervision ratio of one (1) adult for every three (3) children.
All children will be wearing grip socks
I have included a $110 deposit to secure my booking. Etranfer, Cash or cheques (payable to YDC). We do not book a party till we receive the deposit and waiver.
I understand that I am to CLEAN the party room and any other places I used before we leave. PLEASE RECYCLE plastic, cardboard and bottles.
NOTICE: THE STAGE and behind the front curtian and the hallway to the man door is OFF LIMITS. PLEASE STAY OUT of those area.
IF I CANCEL my party less than 48 HOURS before it begins, FOR ANY REASON (including sickness) I will forfeit my deposit.
Dreamland, a family play places, requests a minimum donation of $110 to help cover the cost associated with hosting your party. This does not include the socks to be purchased for your event. If this is a concern please speak to the admins.
I declare that the DreamCentre Yorkton Inc (Church) INC. (members, congregation leadership, and staff) and the District of the PAOC WILL NOT BE HELD RESPONSIBLE FOR ANY INJURIES OR ACCIDENTS WHICH may occur when children in my care are in attendance at DreamCentre or DreamLand.
THE rules for the use of DreamLand need to be enforced by your party please. This includes using bouncer entry points and the toys for the proper use (ie not as bumper cars) etc.
